NO SHOW POLICY

For Office Visits & Office Procedures 

 

This Policy applies to new and established patients. 

 

Our office will call you one day prior to confirm your appointment. If you cannot keep this appointment, you have the option to call our office after hours and leave a message. This will be the only exception to waiving a no-show fee. Farmington ENT & Allergy imposes the following policy with regard to patients who fail to keep their scheduled appointments. 

 

• Patients who fail to show for their scheduled Allergy Testing or VNG Testing appointment or did not notify the office of their scheduled appointment time, will be  subject to a “No Show” penalty of $50.00.   

 

• Patients who fail to show for any other appointment or did not notify the office of their scheduled appointment time, will be subject to a “No Show” penalty of $35.00.
